Open Monday to Friday, 9:00 AM to 5:00 PM. Call (540) 288-2800 and tell us what you need, or send the form on this page and we will call you back.
Call during office hours and you reach someone who can look at the schedule while you are still on the line. That is faster than anything else on this page, and it is how appointments get set.
The form on this page is a request for a call back, not a booking. Send it with a phone number and a sentence about what you need, and we will get back to you, usually within one business day.

Before you call
You do not need to know which of these you are before you dial. But if you would rather read the answer than ask for it, here it is.
New here
Say that when you call and the first appointment gets built around it. Records, the exam and a cleaning are booked together, in one visit, rather than split across two. If you would rather know what you are walking into first, here is what a first visit involves and what to bring.
Coverage
Have the card in front of you when you call. It names your insurer, but it does not say what your particular plan pays for, because employers buy different versions of the same product. So we check your benefits with the plan directly instead of guessing from the card. We accept Virginia Medicaid as well. How coverage gets checked here goes through it step by step.
Something hurts
Call, rather than sending the form. Tell us what is going on, we will talk it through with you, and we will get you in as soon as the schedule allows. If swelling is making it hard to breathe or swallow, or bleeding will not settle, a hospital comes first and we pick it up afterwards. What to do for a knocked-out tooth, a broken tooth or an abscess covers the rest.
